Human-in-the-loop in AI agents means a person reviews, approves, or corrects the agent at specific points before the system takes action. In lending, it is the control layer that keeps an AI agent from making a credit, compliance, or customer-impacting decision on its own when the risk is too high.
How It Works
Think of it like a loan officer working with an analyst.
The AI agent does the first pass: it reads documents, extracts income, checks policy rules, flags anomalies, and drafts a recommendation. The human steps in only at decision points where judgment matters, such as borderline credit quality, missing documents, identity mismatches, or adverse action reasons.
A simple flow looks like this:

- •AI agent ingests bank statements, pay stubs, and bureau data
- •Agent scores risk and checks policy rules
- •If confidence is high and the case is low-risk, it can auto-complete approved steps
- •If confidence is low or a rule is triggered, it routes to a human reviewer
- •Human approves, edits, rejects, or requests more information
- •The final action is logged for audit and model improvement
The analogy I use with lending teams is a teller line with exception handling.
Most transactions are routine. You do not want a manager reviewing every cash deposit or balance inquiry. But if a transaction looks unusual, exceeds limits, or conflicts with policy, it gets escalated. Human-in-the-loop works the same way: automate the boring cases, escalate the risky ones.
For engineering managers, the key design question is not “Should there be a human?” It is “Where exactly does the human sit in the workflow?”
Common control points include:

- •Post-action review: human audits sampled decisions after execution
- •Exception handling: human only sees cases outside policy thresholds
- •Dual approval: two humans must sign off on high-value or regulated actions
In lending systems, you usually want different thresholds for different outcomes. A bot can summarize documents with no issue. It should not independently decline a borrower without traceable rules and reviewer oversight.

If you are managing engineers, this also changes system design. You need queues, role-based access control, audit logs, SLA timers, escalation paths, and clear ownership of final decisions.
Real Example
A mortgage lender uses an AI agent to pre-underwrite applications.
The agent pulls data from uploaded pay stubs, tax returns, and bank statements. It extracts income consistency, detects large unexplained deposits, checks debt-to-income ratio against policy rules, and drafts an underwriting summary for each file.
Here is where human-in-the-loop comes in:
- •If income verification matches across sources and policy checks pass cleanly, the file moves to an underwriter for quick sign-off
- •If bank statement deposits are inconsistent with stated employment income, the case is routed to a senior underwriter
- •If identity verification fails or documents look altered, the agent blocks automatic progression and creates an exception task
- •The underwriter reviews evidence inside one workflow screen and either approves with conditions, requests more docs, or declines
This setup gives the lender speed without giving up control.
The AI handles repetitive extraction work that used to burn underwriter time. The human handles judgment calls where policy interpretation matters. Every override is stored with reason codes so compliance can review outcomes later and product teams can see which parts of the workflow need improvement.
That is the practical pattern: let AI do structured work; let humans handle ambiguity and exceptions.
